Annual Report

Annual report of realization of individual training program and results and progress of work included in the individual research plan for the academic year, including an opinion of supervisor or supervisors, should be submitted by a doctoral student, in hard copy and electronic forms, to the administration of the Doctoral School at appropriate dates of each academic year. 


Annual report is completed by the USOSweb system - "COMMON SECTION" tab and then in the "Applications" tile.


Activities:
1) complete all required fields in the report,
2) attach the opinion of supervisor or supervisors (signed by hand - scan or signed electronically – pdf.),
3) finally save the report
4) generate the report in pdf format:
NOTE - if there is a "draft" watermark on the generated document, it means that the report has not been finally saved,
5) submit the report (together with the opinion of the supervisor or supervisors) to the administration of the Doctoral School:
a) if the report is signed by hand, it must be printed, obtained signatures and the original delivered (scans of documents sent by e-mail will not be accepted),
b) if the report is signed electronically, the generated and signed file should be delivered to the address phd.school@put.poznan.pl.

It is unacceptable to use two ways of signing on one document.


The Report template attached here is only for checking what fields will be required to be completed in USOSweb.
